Public Services Request Form

The DPW Director or designee is to communicate with the Custodial Supervisor to determine maintenance timeline, based on the priority level.

All “Critical” or “Important” priority levels must also be called-in by phone to the DPW Director or designee for immediate determination.

PRIORITY LEVEL

P1 - CRITICAL service response within four (4) hours

P2 - IMPORTANT service response within twenty-four (24) hours

P3 - NORMAL service reply within three (3) days

P4 - LOW service reply within five (5) days
